ABSTRAK

     Efluvium anagen adalah kerontokan rambut yang terjadi pada folikel rambut anagen dan sering disebabkan oleh kemoterapi dan radiasi. Penyebab lain, misalnya logam berat, tanaman, serta penyakit sistemik juga telah dilaporkan. Pada efluvium anagen, kerontokan terjadi sangat banyak, mencapai 90% dari seluruh rambut kepala sehingga dapat berdampak sangat negatif bagi pasiennya terutama jika ditinjau dari aspek psikologis. Mekanisme biomolekular yang terjadi pada efluvium anagen sampai saat ini masih terus diteliti, dan protein apoptosis p53 diduga sangat berperan. Perlu dilakukan anamnesis, pemeriksaan fisis, dan pemeriksaan penunjang yang tepat untuk menyingkirkan berbagai diagnosis banding efluvium anagen, seperti efluvium telogen, alopesia androgenika, sindrom loose anagen, dan alopesia areata tipe difus. Modalitas terapi juga masih terbatas, karena patogenesis efluvium anagen belum terungkap secara tuntas. Minoksidil dapat diberikan sebagai terapi lini pertama dan pasien diberikan informasi cara merawat rambut yang benar ketika rontok. Berbagai metode dengan menggunakan obat biologis untuk mencegah efluvium anagen masih terus dikembangkan dan diharapkan dapat memberikan hasil di masa akan datang (MDVI 2011; 38/1:41-48)

ABSTRACT

     Anagen effluvium is the loss of anagen hair follicles, classically caused by chemotherapy and radiation therapy. Other causes such as heavy metals, plants, and systemic diseases have also been reported. The hair loss in anagen effluvium can be really severe, accounting 90% of the total hair of the scalp. Because of the massive hair loss, it will give negative psychologic impact for the patient.The biomolecular mechanism underlying the process is still being investigated, and the apoptotic p53 protein is suspected to have pivotal role. Careful history taking, physical examination, and supportive laboratory findings are neccessary to exclude the differential diagnosis, such as telogen effluvium, androgenic alopecia, loose anagen syndrome, and diffuse type of alopecia areata. Treatment modality is limited because the pathogenesis is still elusive. Nevertheless, minoxidil can be given as first line therapy along with information on how to manage the hair during hair loss. Many methods preventing the hair loss using biologials are being developed and promising results are to be expected in the near future. (MDVI 2011; 38/1:41-48)
