Kasus: Wanita berusia 20 tahun menderita hipopigmentasi  pasca inflamasi di lengan kiri bawah.  Telah diobati selama tiga bulan dengan krim malam serta krim pagi, namun bercak hipopigmentasi bertambah banyak dan menyebar ke lengan kanan. Infeksi jamur dapat disingkirkan. Pasien kemudian diobati dengan prinsip skin health restoration berupa terapi topikal campuran krim hidrokuinon 4% :  krim tretinoin 0,1% = 1:1 , pelembab, tabir surya SPF 20 dan dilakukan peeling kimiawi dengan asam glikolat sebulan sekali dengan konsentrasi 35% selama 3 kali, lalu meningkat menjadi 50%, juga diberikan vitamin dalam bentuk ester C 1 x 500 mg dan vitamin E 2 x 200 UI sehari. Setelah 4 bulan, bercak hipopigmentasi  memudar.

Diskusi: Skin health restoration ialah konsep yang dikemukakan oleh Obagi,  bertujuan mengembalikan dan menjaga setiap sel kulit dapat melaksanakan fungsinya dengan baik sehingga didapatkan kulit yang sehat. Konsep ini menggunakan dua prinsip, yaitu koreksi kulit yang terjadi di epidermis dan  stimulasi kulit pada dermis papilar dan stratum basal epidermis. Bahan yang digunakan adalah tretinoin (0,05% atau 0,1%), asam α- hidroksi (6-10%), phytic acid (2-10%), dan hidrokuinon (2-4%). Penatalaksanaan dengan konsep skin health restoration pada pasien ini memberikan hasil memuaskan.

Case report:  A twenty year old woman suffered from post-inflammatory hypopigmentation on her lower left arm. The lesion was treated for 3 months with a night cream and day cream. However, the hypopigmentation increased in number , extending to the other arm. Fungal infection was excluded. The patient was treated with skin health restoration program with hydroquinone 4% cream: tretinoin 0,1% cream = 1:1, moisturizer, SPF 20 sunblock gel, and monthly chemical peeling with glycolic acid 35% for 3 times and increased to 50%. We also gave dailiy  vitamin ester C 1 x 500 mg and vitamin E 2 x 200 IU. After 4 months, the hypopigmentation started to fade.

Discussion: Skin health restoration is a concept proposed by Obagi, aimed to restore and maintain skin cells to conduct its function properly in order to achieve healthy skin. This concept is accomplished utilizing two principles; correction; which works on the epidermal layer and stimulation; works on the papillary dermis and the basal layer of the epidermis. Agents used in this concept are tretinoin (0,05% or 0,1%), a hidroxy acid (6-10%), phytic acid (2-10%),and hydroquinone (2-4%). Skin health restoration treatment on this patient gave satisfying result.
