ABSTRAK

     Pakionikia kongenital (PK) merupakan kelompok kelainan displasia ektodermal. Berdasarkan kelainan klinis dan defek keratin yang terjadi, PK dibedakan menjadi pakionikia kongenital tipe-1 dan tipe-. Penyakit ini ditandai dengan distrofi kuku yang hipertrofi. Kelainan ini jarang terjadi, sejak tahun 1904-2005 telah dilaporkan 457 kasus di berbagai literatur.
     Dilaporkan satu kasus PK tipe-1 pada seorang anak laki-laki berusia 7 tahun dengan semua kuku tangan dan kaki yang menebal sejak lahir. Diagnosis ditegakkan berdasarkan gambaran klinis adanya hiperkeratosis subungual pada seluruh kuku tangan dan kaki, leukokeratosis pada tepi lidah, keratosis interfolikular pada lutut dan bokong. Foto rontgen ekstremitas tangan dan kaki dilakukan guna mengetahui adanya komplikasi pada pasien ini memberikan hasil dalam batas normal. Pemberian bahan keratolitik topikal hanya memberikan perbaikan klinis minimal.
     Pemeriksaan genetik dengan polymerase chain reaction untuk mengetahui secara pasti defek gen keratin perlu dilakukan, namun hal ini belum dapat dilaksanakan.(MDVI 2011; 38/1:17-21)

ABSTRACT

     Congenital pachyonychia is a group of ectodermal dysplasia disorder, which differentiated into congenita pachyonycia type-1 and type 2 based on clinical appearance and keratin defect. The major feature of this disorder is hypertrophic nail dystrophy. Congenital pachyonycia is a rare disorder, and has been report about 457 cases in many literature from 1904 to 2005.
     A seventh year old boy with thickening of all toe and finger nail since born, was diagnosed with congenital pachionychia type-1. Diagnosis was established by clinical appearance with subungual hyperkeratosis, leukokeratosis of tongue, interfolicullar keratosis of knee and buttock. The x-ray taken from hand and feet that have been done to exclude the complication are in and the result normal limit. The topical keratolytic only gave a little clinical improvement.
     Genetic examination using polymerase chain reaction should be performed to locate exact defect of keratin gene, unfortunately this examination is not available in our hospital.(MDVI 2011; 38/1:17-21)
